Most dizziness is a problem of the inner ear (peripheral), but central dizziness caused by the brain, such as a cerebellar or brainstem stroke, is an emergency. If, along with dizziness, you have any one of the following, you must go to the emergency room without delay: (1) weakness or sensory abnormality in one arm and leg, (2) double vision in which objects appear doubled, (3) speech difficulty where your articulation is slurred or words will not come out, (4) the most severe headache of your life, or (5) staggering so badly that you cannot walk. In such cases, brain imaging is needed immediately.
