Q During a vertigo attack, the vomiting is severe and it lasts more than twelve hours. What are the criteria for going to the emergency room?
A A Meniere's attack usually subsides within 20 minutes to 12 hours. If it lasts more than 12 hours or is accompanied by one-sided paralysis, double vision, or a speech disturbance, you must go to the emergency room at once to rule out a central cause such as a stroke. Also visit the hospital if severe vomiting raises concern about dehydration.
Detailed Answer
A Meniere's attack usually subsides within 24 hours at most. In the following situations, you must visit the emergency room immediately: ① vertigo lasting more than 12-24 hours or progressively worsening; ② paralysis or sensory abnormality in one arm or leg; ③ double vision in which objects appear doubled; ④ slurred speech or inability to speak; ⑤ extreme headache (the worst of your life); ⑥ a dehydrated state in which severe vomiting prevents any fluid intake. In these cases, neurological evaluation such as brain MRI or CT is needed immediately.
Korean Medicine Clinic Perspective
At Dongjedang, we place patient safety first. As a rule, Korean medicine treatment is started during the stable period after the above warning signs are absent and Meniere's disease has been diagnosed by an otolaryngologist. During the stable periods between attacks, we address phlegm-fluid and Kidney deficiency to reduce the intensity and frequency of the next attack, with the long-term goal of preserving hearing.
