The 12-Week Diet is Not the End — It's Simply an Opportunity to Meet Your Constitution and Your Heart

"I've tried diet herbal medicine a few times.
I lose weight while taking it, but it comes back when I stop."

I am Choi Jang-hyuk, director of Dongjjedang Korean Medicine Clinic.
This is the most frequently heard statement in my practice room, and the one I need to explain most thoroughly.

Our clinic's 12-week detox diet program is not a course for losing weight.
It is not a course, but rather an opportunity.

It is an opportunity to do a thorough cleansing of your deteriorated body and, in that emptied space, to see yourself for the first time.
What kind of constitution do you have, what foods made your body heavy, and where did your heart waver and cause you to reach for food — In Korean medicine, we call this the clarity and turbidity of the heart's flow (心之淸濁).

Therefore, the end of the 12 weeks is not the end.
The real game is the lifetime after the 12 weeks.
Learning what foods suit your constitution, understanding and managing the flow of your heart — this is the real game of dieting.

In this article, I will explain what happens inside your body during these 12 weeks, why that time is not a course but an opportunity, and what should remain after that.

✅ Action | Immediate Practice

Before starting herbal medicine, here are three things you can do to first connect with your body and mind.

1️⃣ Stop eating salty foods for just 1 week
If you cut off ramen, soups, stews, and salty preserved foods for just 1 week, you will feel the puffiness in your face and fingers disappear within the first 3 days.
At the same time, you will lose 1-2kg in body weight.
This is not fat loss.
It is the loss of excess body water.
However, it is a starting point.
Experiencing how sensitive your taste buds, which have become numb to saltiness, become in just 1 week is the first step of self-observation.

2️⃣ Consistent time, consistent amount
Skipping breakfast, binge eating at lunch, eating a lot late at night — this pattern is the real cause of weight gain.
This is because your stomach stretches and shrinks to different sizes every day.
Try maintaining the same time and same amount for just 1 week.
Reducing the amount comes after that.
And after 1 week, observe which time of day your stomach finds most difficult.
That is a clue to your constitution.

3️⃣ Stop for 30 seconds after eating
After eating, pause in that spot for just 30 seconds.
"Why did I just eat that?" — Was it because you were hungry, because it was meal time, because of stress, because you felt lonely, or simply because it was there?
This small question is the real beginning of the 12 weeks.
Observing where your heart leads your hand to food — this is something you can do even before starting constitutional herbal medicine.

If after trying these three things for 1-2 weeks you still feel stuck, then visit the clinic.

🚨 Warning | Critical Warning Signs You Must Check

Constitutional herbal medicine is a safe prescription, but the following people must inform us during consultation.

✔ Those with reduced kidney function or undergoing dialysis
Prescriptions that remove body water put extra work on the kidneys.
For those with weak kidney function, the prescription itself must be different.
Bringing your glomerular filtration rate (GFR) numbers will provide more accuracy.

✔ Those with heart failure or taking heart medications (including diuretics)
If you are already taking diuretics, using herbal medicine together may cause electrolyte imbalance.
Rather than stopping Western medication, we adjust the prescription so you can proceed safely together.

✔ Those who are pregnant or breastfeeding
During this period, tonifying prescriptions take priority over draining prescriptions.

✔ Those in the active phase of hyperthyroidism
If you add draining prescriptions when basal metabolism is already elevated, it becomes a burden.
It is safer to start after thyroid levels stabilize.

🧠 The Why | Root Cause Analysis

1) Why does diet herbal medicine cause weight gain again when stopped?
First, let me clarify the most common misconception.
Most diet herbal medicines work primarily through appetite suppression and strong diuresis.
While taking the medicine, body water is eliminated and weight drops rapidly, but when you stop the medicine, the lost body water returns.
The loss was not fat in the first place.
And fat cells have one important characteristic.
The number of fat cells is determined before puberty, and all subsequent dieting is about reducing their size, not their number.
However, fat cells have a very strong ability to retain water (water-holding capacity).
When weight increases — body water increases first, then fat increases.
When weight decreases — body water decreases first, then fat decreases.
Everyone has experienced eating just one bowl of ramen at night, which weighs less than 1kg, but waking up in the morning 2-3kg heavier.
The ramen didn't turn into fat in between.
The fat cells simply retained water and increased in volume.
The goal of constitutional herbal medicine is not to suppress appetite, but to regulate body water while maintaining satiety.
This way, muscles are preserved and fat cells genuinely shrink in size.

2) Why is starving not effective?
If you excessively reduce food intake, your weight may drop in the short term.
However, what comes off next is muscle.
When muscle is lost, basal metabolic rate decreases, and a body with reduced basal metabolic rate stores more even when eating the same amount.
It's like when a car engine breaks down and loses power output — even with the same fuel, it cannot burn it all and some is left over.
This is why starvation dieting creates a weight-gain body rather than losing weight.
That is why we maintain a consistent amount of food intake.
Preventing muscle loss is a prerequisite for cleansing.

image.png3) 12-Week Cleansing — What Happens in Five Stages
We do not use the same herbal formula throughout the 12 weeks.
The prescription changes through five stages.

① Diuretic Phase (First 2-3 weeks)
Excess body water is eliminated first.
This is when the previously blocked pathways of bowel movements, urination, and perspiration open.
Weight drops relatively quickly, but most of it is body water.
Some people experience heart palpitations or insomnia due to blood pressure fluctuations, which we manage with detox supplements.

② First Weight Loss Phase
Only after the body water is eliminated do fat cells begin to genuinely shrink in size.
This is when clothes become loose.

③ Plateau Phase (1-3 weeks)
After the first weight loss phase, almost everyone experiences a plateau.
This phase is not a standstill.
According to InBody analysis, body fat continues to decrease, but muscle mass recovers and increases, causing body weight numbers to remain stable.
The longer the plateau lasts, the weaker the physical condition or the less muscle mass you have.
If you become impatient here and stop the medicine or start starving, the flow you've built up collapses.
Understanding that the plateau phase is a time of recovery, not endurance, is the most important lesson of the 12 weeks.

④ Second Weight Loss Phase
After passing through the plateau, weight loss resumes.
It drops more slowly than the first phase, but this time real fat is being reduced.

⑤ Completion Phase
A period to stabilize the results accumulated over 12 weeks.
Your reduced stomach size and more sensitive taste buds settle into your body.

4) Different diseases appear the same, but the cause is always the same
From here on is actually the core of this article.

The complaints of those who visit the clinic are all different.
Some come saying they have obesity, some saying they have reflux esophagitis, some saying they have headaches, some saying they cannot sleep.
The diagnoses are different, but the cause ultimately lies in one place.

It is the result of either eating foods that don't match your constitution for a long time, or living for a long time in a place that conflicts with your heart's natural flow.
In Korean medicine, we call this the clarity and turbidity of the heart's flow (心之淸濁).

Each person's heart wavers at a different point, and the scars of that wavering remain in the body.
For some, it manifests as obesity; for others, as reflux esophagitis; for still others, as headaches or insomnia.

Therefore, if you try to address only obesity, it won't be resolved.
You must see the whole person to resolve obesity.

This is why Dongjjedang addresses both constitutional herbal medicine and constitutional diet together in diet treatment.

5) The 12 weeks is an opportunity; what comes after is the real game
The meaning of the 12 weeks has two aspects.

First, cleansing.
We empty the excess substances in your deteriorated body — body water, fat, food residue, and blood stasis — once and for all.
Slowly through five stages, without muscle damage.
