A normal ECG and normal cardiac enzymes in the emergency room rule out an acute event such as myocardial infarction. However, exertional angina can be normal at rest and appear only on movement, so we recommend a cardiology re-evaluation if any of the following apply.
- Tightness reproducibly appears when walking or climbing stairs and eases within minutes of rest
- You have several risk factors such as high blood pressure, diabetes, high cholesterol, smoking, or family history
- The intensity or character of the symptom has clearly changed recently
Conversely, if it recurs only at rest or in specific situations such as meetings, the subway, or bedtime, and lasts for hours, an autonomic explanation fits better. And repeatedly seeking more tests to reassure yourself can itself feed the anxiety and prolong the symptom.
