Q I have a chocolate cyst (endometrioma) along with severe menstrual pain. Can the menstrual pain be managed too?
A Menstrual pain caused by an endometrioma is often much more severe than ordinary menstrual pain. Managing the pain from pelvic inflammation and adhesions, along with the cyst itself, is one of the important goals of Korean medicine treatment.
Detailed Answer
An endometrioma (chocolate cyst) is an ovarian manifestation of endometriosis, where blood inside the cyst irritates the area with each period, causing extreme menstrual pain and pelvic pain. Unlike simple functional menstrual pain, when adhesions form between the uterus, ovaries, and intestines, pelvic pain persists even outside of menstruation, and pain during intercourse and bowel movements may appear. In many cases pain cannot be controlled with painkillers alone, so combined management including hormone therapy and Korean medicine treatment is needed. It is important to first confirm the cyst's size and the extent of adhesions with ultrasound and MRI.
Korean Medicine Clinic Perspective
At Dongjedang, we view menstrual pain caused by an endometrioma as blood-stasis pain (hyeoleo-tongjeung), where congealed blood stasis blocks the pelvis and creates pain. We use acupuncture to help with immediate pain relief, and herbal medicine that clears blood stasis and warms the cold pelvic environment to manage the underlying basis of recurring pain. Many patients from the Dong-gu, Incheon area who suffer from severe menstrual pain come to see us.
