Q I am a 45-year-old homemaker. I feel fine during the day, but when I lie down to sleep my chest tightens and I have to sit up again. Why is it worse at night?
A During the day your attention is spread across tasks, but lying down creates the perfect conditions to focus on bodily sensations. Add factors such as acid rising when reclined or breathing becoming shallow, and the tightness stands out more. Being worse at night is not by itself a dangerous sign.
Detailed Answer
Several factors overlap at night. First, daytime distractions disappear when you lie down and body sensations move to the foreground. Second, as the tension of the day unwinds, arousal levels fluctuate and you become more sensitive to heartbeat and breathing. Third, lying flat makes it easier for stomach contents to move up the esophagus, which can feel like burning tightness between the breastbone and the upper abdomen.
Once anticipatory anxiety sets in, going to bed itself becomes a cue for tension and the symptom becomes fixed.
By contrast, having to sit up because you cannot breathe when lying down, together with swollen ankles, is a different matter. That calls for cardiac and pulmonary evaluation first.
Korean Medicine Clinic Perspective
Chest congestion and difficulty falling asleep at night are seen as energy that rose during the day failing to settle at night. In people with slow digestion, the evening meal may also remain in the upper abdomen and press upward against the chest. In consultation we adjust the timing and size of the evening meal along with pre-sleep habits, and treat in the direction of clearing heat and allowing sleep to arrive.
