#Causes Explained

Q I am 41, office work, sitting more than ten hours a day. For a few weeks I have had a dull ache below my left chest. Could this be my heart?

A Chest pain that begins after long hours of sitting often comes from where the ribs meet the breastbone, and from tension in the back and shoulders. Because heart causes must be ruled out first, it is safer to have basic tests such as an ECG before drawing conclusions. If those tests are normal and the pain changes with posture, a musculoskeletal origin is likely.

Detailed Answer

When the shoulders roll forward and the upper back rounds for long stretches at a desk, the muscles at the front of the chest shorten and stiffen while the upper back muscles hold on in a lengthened position. Over time this imbalance reduces how far the ribs can move, and strain accumulates at the cartilage joining the ribs to the breastbone, appearing as a dull ache or a sharp stab.

This kind of pain typically comes and goes over seconds to minutes, and changes when you twist, reach out an arm, or take a deep breath. Pain from the heart, by contrast, tends to appear regardless of posture and to persist with a squeezing quality. The two can look similar from the outside but differ in character, so a musculoskeletal conclusion should only be reached after the heart and lungs have been checked.

Korean Medicine Clinic Perspective

Korean medicine views chest pain not only as a problem of the muscles and ribs but as a matter of circulation and tension throughout the upper body. We release the stiff areas of the neck and back with acupuncture, and add moxibustion where the area feels cold and tight. For those whose digestion is often heavy or whose upper body stays knotted from stress, we also consider herbal medicine matched to the individual constitution to lower overall tension.
